How do I get a copy of my medical records?

​

You can usually get a copy of your medical record at no charge by going to your doctor’s office, filling out a medical release form and showing ID. Some offices will even allow you to request medical records through the patient portal on your phone or computer.

 

What if I want to have my records faxed or mailed instead of uploading them?

 

You will still need to complete the forms for your desired service online. Make sure the box in your form labeled "Mail or fax instead" is checked.
